Filling out this form is necessary for the conclusion of a contract for the provision of accommodation services between the guest and the accommodation provider Theresian Apartment, i.e., the company Hotel U Terezské brány s.r.o., registered at Javoříčská 674/5, Olomouc, 77900, Company ID No.: 04425341 (hereinafter referred to as the “Controller”).
By completing and signing this form, the guest acknowledges that the Controller is entitled, under the conditions set out below, to process all personal data stated in this form or otherwise obtained directly from the guest in accordance with the applicable legal regulations, in particular Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data (hereinafter referred to as “GDPR”).
The purpose of processing this data is primarily the conclusion and performance of the accommodation services agreement with the Controller (legal basis under Article 6(1)(b) of the GDPR).
Personal data may also be processed for the assertion of possible claims of the Controller arising in connection with the performance of the said contract (in particular for the recovery of unpaid services or compensation for damages) and, together with records from security cameras monitoring selected common areas of the accommodation facility and its immediate surroundings, may also be processed to ensure the safety of guests, their property, and the property of the hotel (legal basis under Article 6(1)(f) of the GDPR).
To the extent and in the manner stipulated by the applicable legal regulations, guests’ personal data will also be processed for the purpose of fulfilling the statutory obligations imposed on the Controller (legal basis under Article 6(1)(c) of the GDPR).
If the guest’s email address is provided, the Controller is also entitled, based on its legitimate interest, to use this data for sending commercial communications (legal basis under Article 6(1)(f) of the GDPR). The guest has the right to refuse such use of their email address at any time and in a simple manner.
The Controller will process personal data for a period of four years from the provision of accommodation services, except for camera recordings, which will be stored for three days, and cases where the law prescribes a longer retention period.
The data subject acknowledges that the Controller is entitled to make the data accessible to contractually authorized processors who operate IT systems used by the Controller in providing accommodation services and who provide accounting and related services to the Controller.
The data subject further acknowledges that they have the right to access their personal data, to request its correction or deletion, as well as the right to request data portability, restriction of processing, to object to processing by the Controller, and, if necessary, to lodge a complaint with the competent supervisory authority, which in the Czech Republic is the Office for Personal Data Protection, all under the conditions set forth in the GDPR.
All the above rights towards the Controller may be exercised by the guest in writing at the address of the hotel, including via email sent to: manager@theresian.cz.
The guest may also contact the Controller in the same manner with any further questions or comments regarding the terms of personal data processing.
By signing the form, the data subject confirms that they have been duly informed by the Controller about the conditions of personal data processing and that all information provided in the form is true and correct.
